Endoscopic therapy (ET) for esophageal varices is technically difficult and is accompanied by complications and high mortality. The purpose of this study was to analyze the safety and efficacy of a modified minimally invasive laparoscopic technique for treatment of ruptured esophageal varices. This is a prospective study. A total of 89 patients with ruptured esophageal varices were treated with endoscopic variceal obturation using Gelfoam (Tyco, United States) and/or N-butyl-2-cyanoacrylate (NBCA; Dermik, France) from 2002 to 2002 and from 2007 to 2009 in our center. In our modified technique, the patients were positioned supine with two laparoscopic ports (12 mm and 5 mm) in the right and left flank and another 5 mm trocar in the left flank for laparoscopic balloon. Vein-ligating sutures were used in two or three layers through the left flank port, over the variceal column and positioned around the sub-diaphragmatic, gastrophrenic and gastro-hepatic ligament. A balloon catheter was inserted through the left flank port and inflated in the gastrophrenic ligament. Accurate placement was confirmed by fluoroscopy. ET was applied in 46 patients (50%) and sclerotherapy in 43 patients (50%). The mean follow-up was 17.2 months. The mean age was 52.7 years (range, 28-68 years). There was no operative mortality. The technical success rate was 95.6% (87/91). Three of the 89 patients (3.4%) were lost to follow-up. Other complications included two patients with chylothorax and two patients with stricture; these resolved with conservative management. Two patients underwent re-laparoscopy because of gastro-esophageal bleeding, but there was no mortality. A modified laparoscopic technique for treatment of ruptured esophageal varices was safe and effective. This technique provided significant benefit by decreasing the complications and mortality associated with ET.Q
